Focus Group Questionnaire

Thank you for your interest in participating in an upcoming focus group! We are seeking a diverse participant group, and are screening submissions for a balanced mix of perspectives. Each group is limited to 12 participants. To begin, please tell us about yourself using the form below. We will let you know if we’re able to include you by the next business day, and will provide the location and other details.

THE BIG PICTURE:

A Vermont organization is seeking your thoughts and opinions on a social issue impacting Vermonters. This is a one-hour commitment, and each participant will receive a $75 Visa gift card at the end of the meeting.
